Walk-On No Insurance Waiver 2014 HIPPA Form Health Information Release Form Walk-On Medical History Physical Sheet
BOWLING GREEN, Ky. -The WKU softball team will hold walk-on tryouts for currently enrolled full-time students on September 3 at the WKU Softball Complex.
A meeting with the Lady Topper coaching staff will begin at 3 p.m. CT at the field before the tryouts begin. You must present a copy of a physical and sickle-cell test taken within the last six months before you will be permitted to participate. Proof of insurance or a waiver of medical insurance, HIPPA (medical release) and a walk-on medical history will also need to be provided. All forms are attached above.
